Triple-Negative Breast Cancer (TNBC): Understanding

Triple-Negative Breast Cancer (TNBC): Overview

Triple-negative breast cancer (TNBC) is a distinct and clinically aggressive subtype of breast cancer characterized by the absence of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2) expression. It represents approximately 10-20% of breast cancer cases and is generally associated with younger age at diagnosis, higher histological grade, increased proliferative activity, early recurrence, and greater metastatic potential compared with several other breast cancer subtypes. Because TNBC lacks the three major therapeutic receptors, it does not benefit from conventional endocrine or HER2-directed therapies, historically leaving chemotherapy as the primary systemic treatment option. TNBC is also highly heterogeneous at the molecular level and includes multiple biological subtypes with distinct gene-expression patterns and therapeutic vulnerabilities, which has increasingly driven efforts toward biomarker-guided and personalized treatment strategies.

The development of TNBC is considered multifactorial and involves a combination of genetic susceptibility, molecular alterations, demographic factors, and other breast cancer risk factors, although a single causative mechanism has not been established. One of the strongest recognized genetic associations is with germline BRCA1 and BRCA2 mutations, particularly BRCA1, which is frequently associated with basal-like and triple-negative tumor characteristics. TNBC is also observed more frequently in younger women and has been reported at higher rates in certain racial and ethnic populations. At the molecular level, alterations affecting DNA-damage repair, tumor-suppressor genes, growth-signaling pathways, and cell-cycle regulation can contribute to malignant transformation and tumor progression. However, these abnormalities vary substantially between tumors, reflecting the heterogeneous nature of TNBC and making it difficult to attribute disease development to a single genetic or environmental cause.

The pathophysiology of TNBC is characterized by substantial molecular heterogeneity, high proliferative activity, genomic instability, and dysregulation of multiple signaling pathways involved in tumor growth, invasion, survival, and treatment resistance. A large proportion of TNBCs exhibit basal-like characteristics, although basal-like breast cancer and TNBC are not completely synonymous. Molecular studies have identified alterations involving DNA-damage repair and BRCA-related pathways, as well as signaling networks such as PI3K/AKT, MAPK, EGFR, androgen receptor, Wnt/B-catenin, and NOTCH. Defects in DNA repair can increase genomic instability and may create specific therapeutic vulnerabilities, while changes in the tumor microenvironment, epithelial-mesenchymal transition, immune signaling, and cancer stem-cell populations can contribute to invasion, metastasis, and resistance to treatment. This complex biology contributes to the aggressive clinical behavior and relatively high risk of recurrence associated with TNBC.

The diagnosis of TNBC begins with the clinical and imaging evaluation of a suspected breast lesion, followed by histopathological confirmation through tissue biopsy. Immunohistochemistry (IHC) is central to classification and evaluates the expression of ER, PR, and HER2; TNBC is generally defined by ER and PR expression of <=1% and absence of HER2 overexpression or gene amplification according to established pathology criteria. Mammography and breast ultrasound are commonly used for initial assessment, while breast MRI can provide additional information regarding tumor extent and treatment response in selected patients. Following confirmation, assessment of tumor size, lymph-node involvement, and distant disease is performed for staging, and additional biomarker or genetic testing may be considered to identify clinically relevant features such as PD-L1 expression or germline BRCA mutations. Because of TNBC's molecular heterogeneity, accurate receptor testing and additional biomarker characterization are increasingly important for treatment selection and prognostic assessment.

Treatment of TNBC is generally multimodal and stage-dependent, incorporating systemic therapy, surgery, and radiation when clinically appropriate. For early-stage disease, chemotherapy remains an important component of treatment and is frequently administered before surgery to reduce tumor burden and assess treatment response, followed by surgery and additional postoperative treatment according to individual disease characteristics. The therapeutic landscape has expanded with the incorporation of immune checkpoint inhibitors, PARP inhibitors for selected patients with germline BRCA alterations, and antibody-drug conjugates and other targeted approaches in appropriate clinical settings. For advanced or metastatic TNBC, treatment selection increasingly incorporates biomarkers such as PD-L1 status, BRCA-related DNA-repair alterations, and other molecular characteristics to identify patients who may benefit from specific targeted or immune-based approaches. Despite these advances, tumor heterogeneity, treatment resistance, recurrence, and limited durable responses in some patients remain important challenges, supporting continued development of biomarker-driven and combination treatment strategies.

It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.

Triple-Negative Breast Cancer (TNBC) Emerging Drugs

Trilaciclib: G1 Therapeutics

Trilaciclib, a transient IV-administered CDK4/6 inhibitor, is a novel therapeutic approach which is given before chemotherapy that temporarily blocks progression through the cell cycle. This provides two potential benefits firstly bone marrow protection and therefore the immune system from damage caused by cytotoxic therapy, and secondly improved long term immune surveillance by upregulating the formation of certain memory T cells. Trilaciclib is being evaluated a number of tumor types and chemotherapy regimens to assess its potential myeloprotection, antitumor efficacy and safety in combination with cytotoxic therapiesand other anticancer agents. Currently, the drug is in the Phase III stage of its development for the treatment of Triple-negative breast cancer.

Dato-DXd: AstraZeneca / Daiichi-Sankyo

Datroway (datopotamab deruxtecan; datopotamab deruxtecan-dlnk in the US) is a TROP2-directed antibody-drug conjugate (ADC) developed using Daiichi Sankyo's proprietary DXd ADC technology. It is one of seven DXd ADC programs in Daiichi Sankyo's oncology pipeline and among the most advanced programs within AstraZeneca's ADC platform. The ADC comprises a humanized anti-TROP2 IgG1 monoclonal antibody, developed in collaboration with Sapporo Medical University, conjugated to multiple topoisomerase I inhibitor payloads consisting of an exatecan derivative (DXd) through tetrapeptide-based cleavable linkers. Currently, the drug is in Phase III stage of its development for the treatment of Triple-negative breast cancer.

BNT327: BioNTech / Bristol-Meyer Squibb

BNT327 is an investigational bispecific antibody designed to integrate two complementary, clinically validated oncology mechanisms within a single molecule. It combines PD-L1 checkpoint inhibition, intended to restore T-cell activity against tumor cells, with VEGF-A neutralization, which aims to alleviate the immunosuppressive tumor microenvironment and inhibit tumor angiogenesis by limiting the blood and oxygen supply required for tumor growth. By targeting PD-L1 on tumor cells, BNT327 is designed to concentrate its anti-VEGF activity within the tumor microenvironment, potentially enhancing treatment precision while reducing systemic VEGF inhibition. The resulting vascular normalization and remodeling may improve tumor perfusion and facilitate the delivery and activity of combination therapies. This dual mechanism and potential to modulate both immune and vascular components of the tumor microenvironment position BNT327 as a potential backbone therapy across multiple solid tumor indications. Currently, the drug is in Phase III stage of it's development for the treatment of Triple-negative breast cancer.

BEBT-209: BeBetter Med

BEBT-209 is an investigational oral, highly selective CDK4 inhibitor developed by BeBetter Med for advanced breast cancer, including metastatic triple-negative breast cancer (mTNBC) and HR+/HER2- advanced or metastatic breast cancer. The molecule demonstrates approximately six-fold greater activity against CDK4 than CDK6 and is designed to inhibit CDK4-mediated cell-cycle progression and tumor-cell proliferation. The asset is currently in Phase II/III development, with ongoing evaluation in combination regimens for treatment of Triple-negative breast cancer.

Evexomostat: SynDevRx, Inc.

Evexomostat (SDX-7320) is an investigational therapy targeting cancer patients with metabolic complications such as obesity, diabetes, hyperglycemia, prediabetes, and insulin or leptin resistance. By irreversibly binding to MetAP2, it modulates key metabolic hormones, lipids, and angiogenic proteins, including bFGF and VEGF-C. Preclinical studies have demonstrated inhibition of cell-cycle signaling, reduced angiogenesis, reversal of obesity-associated immune suppression in the tumor microenvironment, and synergistic antitumor activity with PI3K inhibition. Evexomostat is being developed in combination with standard-of-care therapies for breast cancer and other tumor types where metabolic dysfunction may contribute to disease progression. Currently, the drug is in Phase II stage of it's development for the treatment of Triple-negative breast cancer.

NECVAX-NEO1: NEC Corporation

NECVAX-NEO1 is based on a clinically validated plug-and-play platform designed for convenient oral administration, with intrinsic self-adjuvanting properties provided by its bacterial carrier. Its straightforward, small-scale manufacturing process enables a short turnaround time, supporting potential use in patients with metastatic disease. The platform can target multiple neoantigens of varying sizes and supports both personalized and shared neoantigen approaches. Following administration, antigen epitopes are presented on dendritic cells via MHC class I molecules, activating antigen-specific CD8+ T cells that recognize and eliminate tumor cells expressing the targeted antigens, thereby driving an antitumor immune response. According to company's pipeline, the drug is in Phase I/II stage of it's development for the treatment of Triple-negative breast cancer.
