ANCA Vasculitis: Understanding
ANCA Vasculitis: Overview
Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a group of rare, systemic autoimmune disorders characterized by inflammation and destruction of predominantly small blood vessels. The three major forms are granulomatosis with polyangiitis (GPA), microscopic polyangiitis (MPA), and eosinophilic granulomatosis with polyangiitis (EGPA). The disease commonly affects the kidneys and lungs, but may also involve the skin, peripheral nerves, upper respiratory tract, and other organs. AAV is strongly associated with autoantibodies directed against proteinase 3 (PR3) or myeloperoxidase (MPO), although some patients may be ANCA-negative.
The exact cause of AAV is not fully established and is thought to involve an interaction between genetic susceptibility, environmental factors, infections, and abnormal immune responses. Genetic associations involving PR3, HLA-DP, CTLA-4, and other immune-regulatory genes have been identified, particularly in GPA. Environmental and infectious factors may trigger or exacerbate disease, with Staphylococcus aureus colonization being implicated in GPA. Certain drugs can also induce ANCA-associated vasculitis, particularly those associated with high MPO-ANCA levels.
AAV develops when immune tolerance to neutrophil antigens is lost, leading to the production of ANCA, primarily anti-PR3 or anti-MPO antibodies. Inflammatory cytokines and other stimuli prime neutrophils, causing PR3 and MPO to become accessible on the neutrophil surface. ANCA then binds to these antigens and activates neutrophils, resulting in degranulation, production of reactive oxygen species, release of pro-inflammatory mediators, and adhesion to vascular endothelial cells. Neutrophil extracellular traps (NETs) and activation of the alternative complement pathway, particularly C5a, further amplify inflammation. The resulting endothelial injury and necrotizing inflammation of small vessels lead to tissue damage and manifestations such as rapidly progressive glomerulonephritis, pulmonary hemorrhage, and systemic vasculitis.
Diagnosis of AAV requires integration of clinical presentation, ANCA serology, laboratory investigations, imaging, and, when appropriate, tissue biopsy. Testing for anti-PR3 and anti-MPO antibodies is central to the evaluation, with PR3-ANCA more commonly associated with GPA and MPO-ANCA with MPA. Evaluation should also assess organ involvement through renal function tests, urinalysis, urine protein quantification, inflammatory markers, and imaging of affected organs. A tissue biopsy can provide histological confirmation, particularly when treatment carries substantial risks. Kidney biopsy may demonstrate necrotizing and crescentic glomerulonephritis, while biopsy of affected tissues in GPA can demonstrate necrotizing granulomatous inflammation. Infectious and other autoimmune conditions that can mimic vasculitis should also be excluded.
Treatment aims to rapidly control active inflammation, induce remission, prevent irreversible organ damage, and maintain long-term remission. Management is generally divided into an induction phase followed by maintenance therapy. For severe or organ-threatening disease, induction traditionally involves high-dose glucocorticoids combined with cyclophosphamide or rituximab. Rituximab is particularly important as a B-cell-depleting therapy and is widely used for both induction and maintenance.

ANCA Vasculitis Emerging Drugs Chapters
This segment of the ANCA Vasculitis report encloses its detailed analysis of various drugs in different stages of clinical development, including phase II, I, preclinical and Discovery. It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.
ANCA Vasculitis Emerging Drugs
BDB-001 is an investigational recombinant humanized monoclonal antibody being developed by Staidson (Beijing) Biopharmaceuticals Co., Ltd. for the treatment of ANCA-associated vasculitis (AAV). It targets complement component C5a, a key mediator of inflammation, thereby inhibiting C5a-mediated signaling and reducing complement-driven inflammatory responses. BDB-001 is being evaluated in clinical development for AAV, with the potential to reduce disease-associated inflammation and tissue damage. The drug is currently in Phase III clinical trial evaluation for the treatment of ANCA-associated vasculitis.
NM8074 (ruxoprubart) is an investigational humanized monoclonal antibody being developed by NovelMed Therapeutics for complement-mediated diseases, including ANCA-associated vasculitis (AAV). It selectively targets complement factor Bb, blocking the alternative complement pathway while preserving the classical pathway. By inhibiting alternative pathway activation, NM8074 is designed to reduce complement-mediated inflammation and tissue damage in AAV. The drug is currently in Phase II clinical trial evaluation for the treatment of ANCA-associated vasculitis.
FT839 is an investigational, off-the-shelf, allogeneic CAR T-cell therapy being developed by Fate Therapeutics for autoimmune diseases, including ANCA-associated vasculitis (AAV). Derived from induced pluripotent stem cells (iPSCs), FT839 is engineered to express dual CARs targeting CD19 and CD38, enabling the depletion of B-lineage cells, plasma cells, and activated immune cells involved in autoimmune disease. By simultaneously targeting CD19- and CD38-expressing cell populations, FT839 is designed to provide broader immune-cell depletion and potentially promote immune reconstitution in AAV. The drug is currently in Phase I/II clinical trial evaluation for the treatment of ANCA-associated vasculitis.
KYV-101 (mivocabtagene autoleucel) is an investigational, autologous, fully human CD19-directed CAR T-cell therapy being developed by Kyverna Therapeutics for B-cell-driven autoimmune diseases, including ANCA-associated vasculitis (AAV). It is designed to recognize and eliminate CD19-expressing B cells, enabling deep depletion of pathogenic B cells and potentially promoting immune reconstitution. KYV-101 is currently being evaluated in Phase I clinical studies for AAV. Currently, the drug is being evaluated in the Phase I/II stage of its development for the treatment of ANCA Vasculitis.
