CGRP inhibitors in Migraine: Understanding

CGRP inhibitors in Migraine: Overview

Calcitonin gene-related peptide (CGRP) inhibitors represent a targeted class of therapies developed for the prevention and treatment of migraine. CGRP is a neuropeptide involved in pain transmission and is released during migraine attacks, where it contributes to dilation of cerebral blood vessels and activation of the trigeminovascular system. Blocking the CGRP pathway can therefore reduce migraine frequency and headache days. CGRP-targeting therapies include monoclonal antibodies such as erenumab, fremanezumab, galcanezumab, and eptinezumab, which act against either the CGRP receptor or the CGRP molecule. The introduction of these therapies provided a more targeted approach to migraine prevention, particularly for patients with frequent episodic or chronic migraine who may have inadequate response or tolerability issues with conventional treatments.

The mechanism of action of CGRP inhibitors in migraine is based on blocking the activity of calcitonin gene-related peptide (CGRP), a key mediator of migraine pain. During a migraine attack, CGRP is released from trigeminal nerve fibers and contributes to activation and sensitization of the trigeminovascular system, promoting neurogenic inflammation and vasodilation. CGRP inhibitors interrupt this pathway either by binding directly to CGRP or by blocking the CGRP receptor, thereby reducing CGRP-mediated signaling and preventing the development or propagation of migraine pain. Monoclonal antibodies such as fremanezumab, galcanezumab, and eptinezumab target CGRP itself, whereas erenumab blocks the CGRP receptor.

Migraine is a complex neurological disorder primarily involving activation of the trigeminovascular system, which plays a key role in generating and transmitting migraine pain. Activation of trigeminal nerve fibers leads to the release of neuropeptides, particularly calcitonin gene-related peptide (CGRP), resulting in vasodilation and activation and sensitization of pain pathways. Increased CGRP signaling contributes to the amplification and persistence of pain during migraine attacks. These mechanisms, together with neuronal sensitization and altered processing of sensory signals, contribute to the characteristic headache and associated symptoms of migraine. The involvement of CGRP in these processes has provided an important biological basis for the development of targeted CGRP inhibitors for migraine treatment.

Calcitonin gene-related peptide (CGRP) has emerged as an important therapeutic target in migraine because of its central role in the mechanisms underlying migraine pain.

During migraine attacks, CGRP is released from trigeminal nerve fibers and contributes to activation and sensitization of the trigeminovascular system, promoting vasodilation and transmission of pain signals. Blocking CGRP signaling can therefore interfere with key processes involved in migraine development. This has led to the development of therapies targeting either CGRP itself or its receptor, providing a more specific approach to migraine management compared with several conventional preventive treatments.

Older classes of CGRP antagonists, known as gepants, ealier were associated with hepatotoxicity and elevated liver enzymes, along with side effects such as paresthesia, headache, visual disturbances, and dry mouth. Newer CGRP antagonists have shown improved safety and tolerability, with reported effects including fatigue, joint pain, infections, and injection-site reactions. Their limited blood-brain barrier penetration may reduce the risk of central nervous system toxicity, while the long-term effects of CGRP blockade on cardiovascular, gastrointestinal, and pituitary functions remain under investigation.

Report Highlights:

  • In August 2026, the New drug application for PRT-064040 nasal spray from Sichuan Purity Pharmaceutical Co., Ltd. was accepted, with the acceptance number CXHL2601048.
  • In June 2026, Sichuan Purity Pharmaceuticals presented Phase I clinical data for its novel small-molecule nasal spray PRT-064040 in American Headache Society.
  • In June 2026, Sichuan Purity Pharmaceuticals announced that preclinical research results of PRT-064040 was presented.
  • In May 2026, Mentari Therapeutics announced that the concurrent private placement was led by Fairmount with participation from Commodore Capital, Deep Track Capital, Janus Henderson Investors, a16z Bio + Health, Venrock Healthcare Capital Partners, Wellington Management, TCGX, Blackstone Multi-Asset Investing, BB Biotech, Farallon Capital, RTW Investments, LP, Vivo Capital, Perceptive Advisors and other leading investment management firms. The company also announced that the Private Placement will result in gross proceeds to the combined company of approximately USD 290 million and is expected to fully fund its operations through 2028, beyond the generation of anticipated key clinical datasets from Mentari's parallel lead programs. These programs include MT-002, a anti-CGRP which target validated, complementary, and orthogonal pathways in migraine pathophysiology.
  • In May 2026, InMed Pharmaceuticals, Inc. announced that it has entered into a definitive merger agreement for an all-stock transaction with Mentari Therapeutics, Inc., a privately-held biotechnology company developing therapies for migraine prevention, Indigo Merger Sub Corp. a wholly-owned subsidiary of InMed, and Indigo Merger Sub II, LLC, a wholly-owned subsidiary of InMed. The merger brings together Mentari's differentiated migraine pipeline with InMed's public market infrastructure, positioning the combined company to expedite the development of new therapies for people living with migraine, a debilitating neurological disorder affecting more than 1 billion people globally. Under the terms of the merger agreement, as of the closing of the proposed merger, the pre-merger InMed shareholders are expected to own approximately 1.51% of the combined company, which is expected to have a pro forma equity value of approximately USD 421.4 million (inclusive of the Private Placement).
