Osteosarcoma: Understanding

Osteosarcoma: Overview

Osteosarcoma is the most common primary malignant bone tumor in children and adolescents, although it can occur at any age. It most frequently arises in the metaphysis of long bones, particularly around the knee, and is characterized by malignant cells producing osteoid or immature bone. The disease is aggressive and has a strong propensity for hematogenous metastasis, most commonly to the lungs. Despite improvements in multimodal therapy, outcomes for metastatic or relapsed disease remain challenging.

The most common presenting symptoms are localized bone pain and swelling, often developing over weeks to months and sometimes initially attributed to sports-related injury or trauma. Pain may worsen with activity, weight-bearing or at night, and patients can develop a palpable tender mass and restricted movement of the adjacent joint. Pathological fractures occur in a minority of patients, while systemic symptoms such as fever and night sweats are uncommon. Respiratory symptoms such as cough or hemoptysis generally suggest advanced pulmonary metastatic disease.

The exact cause of osteosarcoma remains largely unknown, but its development involves complex genetic alterations affecting pathways regulating cell growth, differentiation and genomic stability. Risk factors include hereditary cancer-predisposition syndromes such as Li-Fraumeni and hereditary retinoblastoma, Paget disease of bone in older adults, and previous exposure to therapeutic radiation. Most cases are sporadic, with no clearly identifiable environmental cause. Genetic susceptibility and additional acquired genomic abnormalities are thought to cooperate in malignant transformation of osteogenic cells.

Diagnosis typically involves plain radiography followed by MRI of the primary site to define local tumor extent, while CT chest and other imaging are used to assess metastatic disease. Definitive diagnosis requires a properly planned core-needle or open biopsy with histopathological evaluation, ideally performed at a specialized sarcoma center.

Treatment is generally multimodal, combining neoadjuvant multiagent chemotherapy, wide surgical resection and postoperative chemotherapy, with limb-sparing surgery preferred when feasible. Regimens commonly incorporate high-dose methotrexate, doxorubicin and cisplatin; targeted therapies, immunotherapies and tyrosine kinase inhibitors are being investigated particularly for relapsed or metastatic disease.

"Osteosarcoma- Pipeline Insight, 2026" report by DelveInsight outlays comprehensive insights of present scenario and growth prospects across the indication. A detailed picture of the Osteosarcoma pipeline landscape is provided which includes the disease overview and Osteosarcoma treatment guidelines. The assessment part of the report embraces, in depth Osteosarcoma commercial assessment and clinical assessment of the pipeline products under development. In the report, detailed description of the drug is given which includes mechanism of action of the drug, clinical studies, NDA approvals (if any), and product development activities comprising the technology, Osteosarcoma collaborations, licensing, mergers and acquisition, funding, designations and other product related details.

Report Highlights:

  • The companies and academics are working to assess challenges and seek opportunities that could influence Osteosarcoma R&D. The therapies under development are focused on novel approaches to treat/improve Osteosarcoma.

Osteosarcoma Emerging Drugs Chapters

This segment of the Osteosarcoma report encloses its detailed analysis of various drugs in different stages of clinical development, including Phase III, II, I, Preclinical and Discovery. It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.

Osteosarcoma Emerging Drugs

Risvutatug rezetecan: GSK and Hansoh BioMedical R&D Company

Risvutatug rezetecan (Ris-Rez; GSK5764227/HS-20093) is an investigational B7-H3-targeted antibody-drug conjugate (ADC) being developed by GSK and Hansoh BioMedical R&D Company for relapsed/refractory osteosarcoma. It comprises a fully human anti-B7-H3 monoclonal antibody linked to a topoisomerase inhibitor payload, designed to deliver cytotoxic activity to B7-H3-expressing tumor cells. It is being evaluated in Phase III stage for the treatment of Osteosarcoma.

OMO-103: Peptomyc

OMO-103 is an investigational first-in-class direct MYC inhibitor developed by Peptomyc for advanced high-grade osteosarcoma. It is a cell-penetrating mini-protein derived from Omomyc that directly inhibits MYC-driven transcriptional activity, aiming to suppress tumor proliferation and survival. OMO-103 is being evaluated as monotherapy in the Phase II OSTEOMYC trial at a recommended Phase II dose of 6.5 mg/kg as a weekly IV infusion. The study is assessing safety and preliminary antitumor activity, with 16-week progression-free survival as the primary efficacy endpoint.

SOT106: SOTIO Biotech

SOT106 is an investigational LRRC15-targeted antibody-drug conjugate (ADC) being developed by SOTIO for advanced or metastatic osteosarcoma and soft-tissue sarcoma. It combines an LRRC15-targeting antibody with an MMAE cytotoxic payload, linked through a beta-glucuronidase-cleavable linker designed to enable tumor-selective payload release. Preclinical studies demonstrated strong antitumor activity, including tumor regression in osteosarcoma models with low-to-medium LRRC15 expression. SOT106 is being evaluated in a Phase I/II first-in-human study in patients with LRRC15-positive disease.

DIT309: Tcelltech

DIT309 is an investigational B7-H3-targeted CAR-T cell therapy being developed by Tcelltech for advanced osteosarcoma and other bone and soft-tissue sarcomas. It is designed to recognize B7-H3, a tumor-associated antigen expressed on sarcoma cells, and trigger targeted T-cell-mediated tumor killing. DIT309 is being evaluated in a Phase I dose-escalation study using intravenous administration, with the study assessing safety, tolerability, MTD and recommended Phase II dose. The trial is recruiting patients with B7-H3-positive advanced sarcomas after failure or intolerance of standard therapies.
