Primary Myelofibrosis: Understanding
Primary Myelofibrosis: Overview
Primary myelofibrosis (PMF) is a rare, chronic myeloproliferative neoplasm (MPN) characterized by abnormal blood-cell production and progressive fibrosis, or scarring, of the bone marrow. The accumulation of fibrous tissue disrupts normal hematopoiesis, reducing the bone marrow's ability to produce healthy blood cells. As the disease progresses, blood-cell production may shift to organs such as the spleen and liver, causing enlargement of these organs. PMF has a variable clinical course, ranging from an asymptomatic condition detected incidentally to progressive disease associated with significant complications and transformation to acute myeloid leukemia.
Many patients are asymptomatic during the early stages, with abnormalities detected during routine blood testing. Common manifestations include fatigue, weakness, shortness of breath due to anemia, and pain or fullness beneath the left ribs because of an enlarged spleen. Patients may also experience early satiety, bone pain, fever, night sweats, unexplained weight loss and reduced appetite. As the disease advances, low blood-cell counts can contribute to recurrent infections, easy bruising and bleeding, while severe splenomegaly may cause abdominal discomfort and impaired quality of life.
PMF develops when acquired genetic changes arise in hematopoietic stem cells, leading to abnormal cell proliferation and the release of signaling molecules that stimulate bone marrow fibrosis. Frequently identified driver mutations affect JAK2, CALR or MPL, which activate pathways involved in blood-cell growth and survival. These mutations are generally acquired rather than inherited, although familial susceptibility can occur in some cases. Abnormal megakaryocytes and inflammatory signaling contribute to collagen deposition in the marrow, impaired blood-cell formation and extramedullary hematopoiesis. The precise initiating cause remains unclear, and disease progression varies according to molecular, cytogenetic and clinical risk factors.
Diagnosis involves a complete blood count, peripheral blood smear, bone marrow biopsy and molecular testing for driver mutations. Typical findings may include anemia, abnormal white-cell or platelet counts, tear-shaped red blood cells and bone marrow fibrosis; testing also helps exclude other myeloid neoplasms.
Treatment depends on symptoms, disease risk and individual clinical characteristics, with observation appropriate for some asymptomatic, lower-risk patients. JAK inhibitors, including ruxolitinib, fedratinib, pacritinib and momelotinib, may help control splenomegaly and constitutional symptoms, while anemia may require transfusions or targeted supportive treatments. Allogeneic hematopoietic stem-cell transplantation remains the only established potentially curative treatment, but it is suitable only for selected patients.

Primary Myelofibrosis Emerging Drugs Chapters
This segment of the Primary Myelofibrosis report encloses its detailed analysis of various drugs in different stages of clinical development, including phase II, I, preclinical and Discovery. It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.
Primary Myelofibrosis Emerging Drugs
Pelabresib: Novartis
Pelabresib is an investigational oral bromodomain and extraterminal domain (BET) inhibitor being developed by Novartis for primary myelofibrosis and other forms of myelofibrosis. It targets BET proteins, particularly BRD4, to modulate the expression of genes involved in inflammation, fibrosis and abnormal blood-cell proliferation. Pelabresib is being evaluated in combination with ruxolitinib, a JAK1/JAK2 inhibitor, to improve spleen volume reduction and disease-related symptoms. The Phase III MANIFEST-3 study is comparing pelabresib plus ruxolitinib against ruxolitinib alone in JAK-inhibitor-naive patients with myelofibrosis.
Nuvisertib: Sumitomo Pharma
Nuvisertib (TP-3654) is an investigational, oral selective PIM1 kinase inhibitor developed by Sumitomo Pharma for primary and secondary myelofibrosis. It targets PIM1, a signaling kinase involved in abnormal blood-cell proliferation, cell survival and disease progression, with the aim of reducing myelofibrosis-associated symptoms and splenomegaly. Nuvisertib is also being investigated in combination with ruxolitinib or momelotinib to improve treatment responses in patients with suboptimal responses to, or resistance to, prior JAK inhibitor therapy. It is being evaluated in an ongoing Phase I/II clinical trial assessing safety, tolerability, pharmacokinetics and preliminary efficacy.
LY5830966: Eli Lilly and Company
LY5830966 (AJ1-11095) is an investigational, oral, first-in-class type II Janus kinase 2 (JAK2) inhibitor being developed by Ajax Therapeutics, a subsidiary of Eli Lilly, for primary and secondary myelofibrosis. It selectively binds JAK2 in its inactive conformation, aiming to suppress aberrant JAK2 signaling that drives abnormal blood-cell production and bone marrow fibrosis. The approach is designed to offer a treatment option for patients who have failed or relapsed after treatment with existing type I JAK2 inhibitors. LY5830966 is being evaluated in a Phase I clinical trial assessing safety, tolerability, pharmacokinetics and preliminary clinical activity.
