MPS-IH is a severe, progressive genetic disorder that causes glycosaminoglycan accumulation and can damage the brain, skeleton, heart, eyes and other organs. MPS-I overall is estimated to occur in approximately 1 in 100,000 live births, and around 60% of affected children have the severe Hurler subtype, according to company-reported disease background. Existing treatments, including enzyme replacement therapy and allogeneic stem-cell transplantation, have limitations, particularly for some neurological and skeletal manifestations. This creates an important need for more comprehensive treatment.
In the initial proof-of-concept study, all eight treated children demonstrated stable cognitive performance, motor function and growth within age-appropriate ranges at multiple post-treatment assessments. Researchers also reported improved joint range of motion and encouraging findings in skeletal, ocular and auditory manifestations. Although the study was small and non-randomized, these findings support further investigation of OTL-203's potential to address several disease manifestations.
OTL-203 has demonstrated supraphysiological IDUA enzyme expression in peripheral blood, alongside reductions in glycosaminoglycans in urine and cerebrospinal fluid. In the original eight-patient study, all patients achieved high IDUA expression at 12 months. These biomarker findings support the therapy's biological activity and could underpin its long-term value if they translate into durable clinical benefits.
OTL-203 uses patients' own hematopoietic stem cells, which are genetically modified outside the body and then reinfused following conditioning treatment. This approach aims to provide long-term production of the missing enzyme. In the initial study, patients discontinued enzyme replacement therapy before gene therapy, and none had restarted it at the reported follow-up. If durable benefit is confirmed, OTL-203 could reduce dependence on repeated ERT and potentially avoid some risks associated with donor-derived transplantation.
Orchard Therapeutics announced on July 7, 2025, that the last patient had been treated in HURCULES, its registrational trial comparing OTL-203 with standard allogeneic hematopoietic stem-cell transplantation. The study planned to enrol 40 patients, randomized equally between the two groups. Its primary endpoint is assessed two years after treatment and includes clinically meaningful outcomes such as survival, treatment failure, immune complications, cognition and growth. Completion of enrolment nearly a year ahead of schedule was an important development milestone.
OTL-203 Recent Developments
In February 2024, Orchard Therapeutics, acquired by Kyowa Kirin with the goal of accelerating the delivery of new gene therapies to patients around the globe, announced the first patient has been randomized at the M Health Fairview Masonic Children's Hospital in a registrational trial evaluating the efficacy and safety of OTL-203, an investigational hematopoietic stem cell (HSC) gene therapy, in patients with the Hurler subtype of mucopolysaccharidosis type I (MPS-IH). The trial, referred to as HURCULES, compares treatment with OTL-203 to standard of care with allogeneic hematopoietic stem cell transplant (HSCT), and is expected to enroll 40 MPS-IH patients at sites across the US and Europe.

OTL-203 Drug Summary
OTL-203 is an investigational, ex vivo autologous hematopoietic stem cell (HSC) gene therapy developed by Orchard Therapeutics in collaboration with the San Raffaele-Telethon Institute for Gene Therapy (SR-Tiget) for the treatment of mucopolysaccharidosis type I-Hurler syndrome (MPS-IH), the most severe form of mucopolysaccharidosis type I. This rare, inherited lysosomal storage disorder is caused by mutations in the IDUA gene, resulting in deficiency of the alpha-L-iduronidase (IDUA) enzyme and progressive accumulation of glycosaminoglycans (GAGs) in multiple organs, including the brain, heart, bones, eyes and respiratory system. OTL-203 involves collecting a patient's own CD34-positive hematopoietic stem and progenitor cells, genetically modifying them outside the body using a lentiviral vector to introduce a functional copy of the IDUA gene, and administering the modified cells back to the patient following a conditioning regimen. The modified cells are designed to produce functional IDUA enzyme, enabling the breakdown of accumulated GAGs and potentially improving metabolic control, neurological development and other disease manifestations. The report provides OTL-203's sales, growth barriers and drivers, post usage and approvals in multiple indications.
