Zilovertamab vedotin targets ROR1, an oncofetal receptor expressed on several hematologic malignancies, and delivers the microtubule inhibitor MMAE directly to ROR1-positive cells. In the first-in-human Phase I study, 7/15 patients with MCL (47%) and 3/5 patients with DLBCL (60%) achieved objective responses despite a median of four prior therapies. These findings provided clinical proof-of-concept for ROR1-directed therapy and established the rationale for subsequent development.
The Phase II waveLINE-007 study is evaluating zilovertamab vedotin + R-CHP in previously untreated DLBCL. At the recommended 1.75 mg/kg dose, all 15 evaluable patients achieved a complete response (100%), while the overall CR rate across all dose groups was 97.2%. The 12-month duration-of-response rate was 93.5%, with median DOR not reached at a median follow-up of 17.6 months. These results led Merck to select 1.75 mg/kg as the recommended dose for Phase III development.
DLBCL is the most common form of non-Hodgkin lymphoma and accounts for approximately 25-30% of NHL cases worldwide. Merck estimates approximately 25,000 patients are diagnosed with DLBCL annually in the U.S. Although frontline treatment with R-CHOP or related regimens produces durable responses for many patients, a substantial proportion eventually relapse or become refractory. Merck has cited an unmet need for additional first-line options because approximately 40% of patients experience relapsed/refractory disease after initial treatment. Successful Phase III development could therefore place zilovertamab vedotin in a large treatment population.
Merck's current pipeline lists zilovertamab vedotin in Phase III hematologic-malignancy development, with programs including waveLINE-003, waveLINE-004, waveLINE-006 and waveLINE-007, alongside additional development through waveLINE-010. This suggests that Merck is pursuing the molecule across multiple lymphoma settings rather than relying on a single indication. Successful results in more than one setting could substantially increase its eventual addressable population.
Zilovertamab vedotin uses an antibody against the extracellular domain of ROR1, linked through a cleavable linker to MMAE. ROR1 is expressed in several malignant B-cell populations while having limited expression in most normal adult tissues, providing a biological rationale for selective tumor targeting. The Phase I study demonstrated exposure-dependent targeting of ROR1 on circulating tumor cells and did not identify unexpected ROR1-mediated toxicity, supporting continued investigation of this mechanism.
Zilovertamab vedotin Recent Developments
In February 2025, Merck, known as MSD outside of the United States and Canada, today announced the initiation of waveLINE-010, a pivotal Phase III clinical trial evaluating zilovertamab vedotin in combination with rituximab plus cyclophosphamide, doxorubicin and prednisone (R-CHP) compared to rituximab plus cyclophosphamide, doxorubicin, vincristine and prednisone (R-CHOP) alone, for the treatment of patients with previously untreated diffuse large B-cell lymphoma (DLBCL). Zilovertamab vedotin is Merck's investigational antibody-drug conjugate (ADC) that targets receptor tyrosine kinase-like orphan receptor 1 (ROR1). Global recruitment of the waveLINE-010 trial has begun, with patients now enrolling.

Zilovertamab vedotin Drug Summary
Zilovertamab vedotin is an investigational antibody-drug conjugate (ADC) developed by Merck (MSD) for the treatment of certain B-cell malignancies. The drug is designed to target ROR1 (receptor tyrosine kinase-like orphan receptor 1), a cell-surface protein expressed on various hematologic cancers and several solid tumors but generally absent or expressed at low levels in most normal adult tissues. Zilovertamab vedotin consists of a monoclonal antibody directed against ROR1 linked to the cytotoxic payload monomethyl auristatin E (MMAE) through a protease-cleavable linker. Following intravenous administration, the antibody component binds to ROR1-expressing tumor cells and facilitates internalization of the ADC; intracellular cleavage releases MMAE, which disrupts microtubule function, causes cell-cycle arrest, and ultimately induces tumor-cell death. The drug has been investigated in clinical studies involving patients with mantle cell lymphoma, chronic lymphocytic leukemia/small lymphocytic lymphoma, diffuse large B-cell lymphoma, and other ROR1-expressing malignancies, including studies evaluating its use alone and in combination with established therapies. Zilovertamab vedotin remains an investigational therapy, with its clinical development focused on determining its safety, antitumor activity, and potential role in treating ROR1-positive cancers. The report provides Zilovertamab vedotin's sales, growth barriers and drivers, post usage and approvals in multiple indications.
