Description
Dedifferentiated liposarcoma is a high-grade nonliposarcoma that arises in conjunction with an earlier, well-differentiated liposarcoma. Primary tumors, or de novo tumors, outnumbered secondary tumors by a ratio of 9:1 and the differentiation phenomenon was time dependent. Men are more likely to develop tumors than women, and the abdominal cavity is where they are most frequently found in adults over the age of 60. Other sarcoma phenotypes are less well understood, but the most prevalent phenotype is histologic characteristics similar to those of malignant fibrous histiocytoma. Less than 5% of all childhood soft tissue tumors are liposarcomas, one of the less common types of non-rhabdomyosarcoma. Large, deep spaces of connective tissue are present in liposarcomas, a specific type of lipoma. Lower extremities, the retroperitoneum, and the shoulder region are the three primary locations for liposarcoma to develop. The popliteal fossa and the inner thigh are the preferred sites for the formation of the lower limbs. The perineum is the region that is most frequently found behind the peritoneum. In the subcutaneous layer of the face, neck, and shoulders, tumors can occasionally form. Children are more likely to develop tumors of the lower extremities.
Advanced unresectable dedifferentiated liposarcoma (Epidemiology)
Liposarcoma patients' survival statistics are frequently determined from small series or data from adults because they are so uncommon. Stage and grade are two prognostic factors that affect prognosis, just like they do for other childhood soft tissue sarcomas. A full surgical resection is essential. Wide excision or adjuvant radiation can reduce the likelihood of local recurrence following resection, which is common. Therefore, compared to a retroperitoneal location, an extremity location that permits aggressive surgery is a favorable prognostic factor. The most typical site of metastatic spread for high-grade pleomorphic tumors is the lung. There hasn't been any evidence of lymphatic spread. Although myxoid liposarcoma is typically regarded as moderate, in 10-35% of patients, it can occasionally spread to soft tissue regions other than the lungs, including the retroperitoneum, chest wall, and even the brain and spine. Less than 5% of soft tissue tumors in children in the US are liposarcomas. Liposarcoma in children has been reported in the literature in fewer than 150 cases overall, most of which develop in the second decade of life. Over a four-year period, 18 cases of liposarcoma in patients 22 years of age or younger were reported at a major cancer hospital in New York City. A multi-institutional study looked at 82 patients who were diagnosed over a 27-year span and were under the age of 22.

Unfortunately, trabectedin hasn't shown any promise in pediatric studies when used alone. Targeted therapy has been determined by a better understanding of the molecular biology of liposarcoma. Promising outcomes have been seen with MDM2 inhibitors like SAR40538 and CDK4 inhibitors like NVP-LEE011. In patients with unresectable advanced dedifferentiated liposarcoma after at least two prior operations, Selinexor (Xpovio) showed a statistically significant improvement in progression-free survival (PFS) compared with matched placebo. by SEAL Experience (NCT02606461). in 2020 at the CTOS Annual Meeting. According to the findings, selinexor had a median PFS of 2.83 months compared to 2.07 months for placebo, which resulted in a 30% decrease in the risk of the disease progressing or ending in death (HR; 0.70; P = 0.023). Additionally, the selinexor group had PFS rates of 23.9% at 6 months and 8.4% at 12 months, versus 13.9% and 2% in the placebo group, respectively.
