Postmenopausal Osteoporosis: Understanding
Postmenopausal Osteoporosis: Overview
Postmenopausal osteoporosis (PMO) is a skeletal disorder characterized by reduced bone mass, deterioration of bone microarchitecture, and decreased bone strength, resulting in an increased risk of fragility fractures. It is particularly associated with the menopausal transition, when accelerated bone loss occurs because of declining estrogen levels. Bone loss is especially important during the early postmenopausal period and contributes to an increased risk of vertebral, hip, and other osteoporotic fractures with advancing age.
The primary cause of postmenopausal osteoporosis is estrogen deficiency associated with menopause, which accelerates bone loss. Additional risk factors include older age, genetic predisposition, low body weight, physical inactivity, smoking, excessive alcohol or caffeine consumption, inadequate calcium and vitamin D intake, and insufficient sunlight exposure. Secondary contributors can include certain systemic diseases and medications that adversely affect bone metabolism. Genetic factors may also account for a substantial proportion of an individual's susceptibility to postmenopausal osteoporosis.
Bone is continuously remodeled through coordinated osteoclastic bone resorption and osteoblastic bone formation. After menopause, reduced estrogen levels increase osteoclast production and activity, resulting in excessive bone resorption that is not adequately compensated by new bone formation. This imbalance causes loss of trabecular and cortical bone, deterioration of bone microarchitecture, and progressive reduction in bone mineral density (BMD). The resulting reduction in bone strength makes postmenopausal women more susceptible to fragility fractures.
Diagnosis of postmenopausal osteoporosis primarily relies on bone mineral density assessment using dual-energy X-ray absorptiometry (DXA/DEXA), particularly at the hip and spine. A T-score of -2.5 or lower is generally used to define osteoporosis, whereas a T-score between -1.0 and -2.5 indicates osteopenia. Assessment of fracture risk can also incorporate clinical factors such as age, previous fractures, smoking, alcohol consumption, glucocorticoid exposure, and BMD using tools such as FRAX, which estimates the 10-year probability of hip and major osteoporotic fractures.
Treatment of postmenopausal osteoporosis combines lifestyle measures, nutritional supplementation, and pharmacological therapy according to fracture risk. Adequate
calcium and vitamin D intake, weight-bearing and resistance exercise, smoking cessation, limiting alcohol consumption, and fall prevention are important components of management. Pharmacological treatments include anti-resorptive agents, such as bisphosphonates and denosumab, which reduce bone resorption, and anabolic agents, such as teriparatide and abaloparatide, which stimulate new bone formation. Menopausal hormone therapy may also help preserve BMD in selected women, particularly around the time of menopause, following individualized assessment of its benefits and risks.

It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.
Postmenopausal Osteoporosis Emerging Drugs
AGA2118 is a bispecific antibody that targets sclerostin and DKK1 and is in clinical development for the treatment of osteoporosis. Sclerostin and DKK1 are two critical, negative regulators of WNT signaling in osteoblasts and of bone metabolism. By targeting both proteins, AGA2118 is thought to prevent compensatory increase of either agent, aiming to improve the magnitude of bone mineral density gains in osteoporotic patients. Angitia is developing AGA2118 in OLYMPUS (Osteoporosis Longitudinal multi-Year prograM to evaluate Postmenopausal women at risk for fractUreS), which is currently under investigation in the ARTEMIS Phase II trial (A Phase II Randomized Trial Evaluating Efficacy of AGA2118 in PostMenopausal Women wIth Low Bone MasS).
HMM-910 is a human umbilical cord-derived mesenchymal stem cell (MSC) therapy developed by HELP Therapeutics for the treatment of postmenopausal osteoporosis. The therapy is administered by intravenous infusion and is being evaluated in postmenopausal women with osteoporosis who are at high risk of fracture. Currently, the drug is being evaluated in the Phase I stage of its development for the treatment of Osteoporosis.
