DISC-0974 is a first-in-class monoclonal antibody targeting hemojuvelin (HJV), a key upstream regulator of hepcidin, the master hormone controlling iron homeostasis. Elevated hepcidin contributes significantly to anemia in myelofibrosis (MF) and other inflammatory disorders by restricting iron availability for erythropoiesis. By inhibiting HJV, DISC-0974 suppresses hepcidin production, increases circulating iron, and promotes red blood cell production through a mechanism distinct from erythropoiesis-stimulating agents (ESAs) or JAK inhibitors. This novel biology positions DISC-0974 to address a substantial unmet need in MF patients, many of whom experience treatment-limiting anemia.
Clinical data presented at the 2023 American Society of Hematology (ASH) Annual Meeting and updated in 2024 showed robust biological and clinical activity. DISC-0974 produced dose-dependent reductions in serum hepcidin exceeding 75% and corresponding >75% increases in serum iron at the 28 mg dose. Among evaluable non-transfusion-dependent MF patients treated at the 28-50 mg dose levels, 57% achieved a hemoglobin increase of >1.5 g/dL. Updated 2024 results further demonstrated that 68.9% of non-transfusion-dependent patients achieved a hemoglobin response >=1.5 g/dL, while one transfusion-dependent patient became transfusion independent and all evaluable transfusion-dependent patients experienced at least a 50% reduction in transfusion requirements. These findings strongly support continued development.
Anemia remains one of the principal limitations of current JAK inhibitors such as ruxolitinib, with many patients requiring dose reductions or discontinuation because of worsening cytopenias. DISC-0974 is being evaluated in patients both receiving and not receiving concomitant JAK inhibitors, offering the potential to improve anemia without compromising MF disease control. Updated clinical data showed hemoglobin responses in patients receiving concurrent JAK inhibitor therapy, supporting its potential use as an add-on treatment rather than a replacement for existing standard therapies. This complementary positioning could significantly broaden its commercial opportunity.
Beyond myelofibrosis, Disc Medicine is evaluating DISC-0974 in non-dialysis-dependent chronic kidney disease (NDD-CKD)-associated anemia, another condition driven by elevated hepcidin levels. The company has stated that suppression of the HJV-hepcidin pathway could be applicable across several chronic inflammatory anemias, substantially expanding the drug's addressable market beyond the relatively small MF population. This lifecycle strategy diversifies development risk while creating opportunities for future label expansion.
Disc Medicine has identified DISC-0974 as one of its core pipeline assets. According to the company's recent development update, the ongoing Phase II study in MF-associated anemia is expected to generate topline data in 2026, and positive results could support discussions with regulatory authorities regarding a registrational development pathway. Progression into pivotal studies would substantially de-risk the program and increase its commercial attractiveness.
DISC-0974 Recent Developments
In June 2026, Disc Medicine, Inc. (NASDAQ:IRON), a clinical-stage biopharmaceutical company focused on the discovery, development, and commercialization of novel treatments for patients suffering from serious hematologic diseases, presented updated data from the RALLY-MF Phase II trial of DISC-0974 in anemia of MF at the ASCO Annual Meeting in Chicago, IL. In this updated data set, treatment with DISC-0974 shows substantial reductions in hepcidin and increases in iron levels translating to positive impact on clinically meaningful measures of anemia across a broad range of patient types.

DISC-0974 Drug Summary
DISC-0974 is an investigational, subcutaneously administered humanized monoclonal antibody targeting hemojuvelin (HJV) that is being developed for the treatment of anemia associated with myelofibrosis and other disorders characterized by ineffective erythropoiesis and iron-restricted anemia. DISC-0974 works by binding to hemojuvelin, a key regulator of the bone morphogenetic protein (BMP)/SMAD signaling pathway, thereby suppressing hepatic hepcidin production, increasing iron availability for erythropoiesis, and improving red blood cell production. Administered by subcutaneous injection, DISC-0974 is being evaluated in early-phase clinical trials both as a monotherapy and in combination with standard treatments, with preliminary data demonstrating increases in hemoglobin levels, reductions in transfusion requirements, and favorable pharmacodynamic effects on iron metabolism in patients with myelofibrosis-related anemia. The therapy has generally shown an acceptable safety profile to date, with most reported adverse events being mild to moderate, including injection-site reactions and transient laboratory abnormalities. Although not yet approved by regulatory authorities, DISC-0974 represents a promising novel approach to addressing anemia in myelofibrosis by targeting iron homeostasis rather than directly inhibiting malignant cell proliferation. The report provides DISC-0974's sales, growth barriers and drivers, post usage and approvals in multiple indications.
