SGLT2 Inhibitors Overview

Sodium-glucose cotransporter 2 (SGLT2) inhibitors act on SGLT2 receptors in the kidneys, which is responsible for reabsorbing glucose from the urine back into the bloodstream.

SGLT2 inhibitors stand out for their ability to provide cardiovascular and kidney protection, in addition to managing blood sugar. Unlike other treatments, they also promote weight loss and help lower blood pressure, offering a comprehensive approach to managing type 2 diabetes and related complications.

Target Patient Pool Analysis

SGLT2 Inhibitors Marketed Drugs

FARXIGA/FORXIGA: AstraZeneca

FARXIGA/FORXIGA (dapagliflozin), developed by AstraZeneca, is indicated in patients with type 2 diabetes mellitus to improve the control of blood glucose in addition to diet and exercise. Other indications include minimizing the hospitalization attributed to heart failure in type 2 diabetes mellitus patients with underlying cardiovascular illness or several cardiovascular risk factors. It also received approval for managing CKD by US FDA in April 2021. In May 2023, FARXIGA had been approved in the US to reduce the risk of cardiovascular death, hospitalization for heart failure and urgent heart failure visits in adults with heart failure.

In June 2024, FARXIGA (dapagliflozin) has been approved by the US FDA to improve glycemic control in pediatric patients with type-2 diabetes aged 10 years and older.

JARDIANCE: Boehringer Ingelheim

JARDIANCE (empagliflozin), developed by Boehringer Ingelheim, and co-market with Eli Lilly. It is used to reduce the risk of cardiovascular death and hospitalization due to heart failure in adults, as well as to reduce the risk of cardiovascular death in adults with type 2 diabetes. Additionally, it helps lower blood sugar in adults and children aged 10 years and older with type 2 diabetes, when used alongside diet and exercise.

In September 2023, the US FDA approved JARDIANCE to reduce the risk of sustained decline in estimated glomerular filtration rate (eGFR), end-stage kidney disease, cardiovascular death and hospitalization in adults with CKD at risk of progression.

SGLT2 Inhibitors Emerging Drugs

YG1699: Youngene Therapeutics

YG1699 is a dual inhibitor of SGLT1/SGLT2. It offers all the benefits of SGLT2 inhibition, while also providing SGLT1 inhibition to enhance efficacy. YG1699 carries unique potential to help patients with type 1 diabetes mellitus better control their blood glucose levels, particularly after meals, and improve their overall quality of life. In addition, the associated benefits of dual SGLT1 and SGLT2 inhibition may reduce the risks associated with diabetes, including cardiovascular disease and diabetic nephropathy.

The company is investigating the drug in various trials, including heart failure, type 2 diabetes, and diabetes with severe CKD in Phase I, and type 1 diabetes in Phase II in China.

The company conducted a Phase I trial for YG1699 in type 1 diabetes in the US, but there have been no further updates on this trial from the company.

The company is now actively involved in planning and developing the clinical research protocol for YG1699 in heart failure study in the US.

The FDA has approved several drugs as single-ingredient products or in combination with other medications, such as JARDIANCE and SYNJARDY (empagliflozin and metformin hydrochloride), STEGLATRO (ertugliflozin) and metformin, and others. Despite a constrained pipeline, some companies are expanding their offerings by combining these approved drugs with other therapies.

These combinations with SGLT2 inhibitors includes:

Zibotentan/Balcinrenone/Baxdrostat and Dapagliflozin: AstraZeneca

A Phase III trial is underway for balcinrenone and dapagliflozin in heart failure with CKD, baxdrostat and dapagliflozin in CKD and prevention of heart failure, and zibotentan and dapagliflozin in CKD with high proteinuria.

Pemafibrate and Tofogliflozin: Kowa Company

The company is conducting a Phase II trial for pemafibrate/tofogliflozin in the treatment of metabolic dysfunction-associated steatohepatitis. Pemafibrate is a selective PPARa modulator, while tofogliflozin is an SGLT2 inhibitor.
