Primary Biliary Cholangitis: Understanding
Primary Biliary Cholangitis: Overview
Primary biliary cholangitis (PBC) is a chronic, progressive autoimmune cholestatic liver disease characterized by immune-mediated destruction of the small intrahepatic bile ducts. Progressive duct injury impairs bile flow, resulting in retention of bile acids and other biliary constituents, chronic cholestasis, inflammation, and ultimately hepatic fibrosis. Although PBC can remain asymptomatic for years, untreated disease may progress to cirrhosis, portal hypertension, liver failure, and the need for liver transplantation. PBC predominantly affects women, although contemporary studies indicate that the disease occurs across sexes and ethnicities.
Many patients are asymptomatic when PBC is diagnosed, often following investigation of abnormal liver biochemistry. Pruritus (itching) and fatigue are the most characteristic and burdensome symptoms and can substantially impair sleep and quality of life. Other manifestations include sicca symptoms, particularly dry eyes and mouth, abdominal discomfort, and complications of chronic cholestasis such as osteoporosis, hyperlipidemia, and fat-soluble vitamin deficiencies. As the disease advances, patients may develop jaundice, hepatosplenomegaly, ascites, variceal bleeding, and other manifestations associated with cirrhosis and portal hypertension.
The precise cause of PBC remains incompletely understood and is believed to involve an interaction between genetic susceptibility, epigenetic factors, and environmental triggers that leads to loss of immune tolerance. T lymphocytes and other immune cells target biliary epithelial cells, resulting in progressive destruction of the small intrahepatic bile ducts. The disease has a strong serologic association with antimitochondrial antibodies (AMA), which are detected in approximately 90-95% of patients. Proposed environmental triggers include infections and exposure to certain chemicals or drugs, although no single trigger has been established as the cause of PBC.
Diagnosis generally requires at least two of three criteria: biochemical evidence of cholestasis, particularly elevated alkaline phosphatase (ALP); positive AMA or PBC-specific antibodies such as anti-gp210 or anti-sp100; and compatible liver histology showing nonsuppurative destructive cholangitis. Liver ultrasound is commonly used to exclude extrahepatic biliary obstruction, while biopsy is generally reserved for diagnostically uncertain cases or suspected overlap with autoimmune hepatitis. Ursodeoxycholic acid (UDCA) remains the standard first-line disease-modifying treatment, while patients with an inadequate response or intolerance may require second-line therapy or clinical-trial options. Management also includes treatment of pruritus and fatigue and monitoring for bone disease, nutritional deficiencies, and progression to cirrhosis.

Primary Biliary Cholangitis Emerging Drugs Chapters
This segment of the Primary Biliary Cholangitis report encloses its detailed analysis of various drugs in different stages of clinical development, including Phase III, II, I, Preclinical and Discovery. It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.
Primary Biliary Cholangitis Emerging Drugs
Volixibat: Mirum Pharmaceuticals
Volixibat is an investigational, orally administered, minimally absorbed ileal bile acid transporter (IBAT) inhibitor being developed by Mirum Pharmaceuticals for cholestatic pruritus associated with primary biliary cholangitis (PBC). By blocking IBAT-mediated reabsorption of bile acids in the ileum, volixibat increases fecal bile acid excretion and reduces systemic bile acid levels. In PBC, it is being evaluated in the Phase IIb/III VANTAGE program, including patients with moderate-to-severe cholestatic pruritus. Volixibat has received US FDA Breakthrough Therapy designation for cholestatic pruritus in patients with PBC.
Golexanolone: Umecrine Cognition
Golexanolone is an investigational GABAA receptor-modulating steroid antagonist being developed by Umecrine Cognition to address neurocognitive symptoms associated with PBC. It is specifically being developed for PBC patients experiencing clinically significant central fatigue and cognitive impairment, symptoms that are not directly addressed by current disease-modifying PBC therapies. Golexanolone is being evaluated in the Phase Ib/IIa study, a randomized, double-blind, placebo-controlled trial assessing safety, pharmacokinetics, and preliminary efficacy. The study evaluates 40 mg and 80 mg twice-daily doses for 28 days, with outcomes including fatigue, daytime sleepiness, cognitive function, and health-related quality of life; the last patient completed follow-up in September 2026, with results expected in November 2026. As per the company's pipeline, the drug is currently in the Phase II stage of its development.
DF5112: Dragonfly Therapeutics
DF5112 is an investigational monoclonal antibody developed by Dragonfly Therapeutics that targets CCR6, a chemokine receptor expressed on Th17 cells and other immune-cell populations. It is designed to deplete CCR6-expressing Th17 cells and B cells, thereby modulating immune pathways implicated in autoimmune and inflammatory diseases. For primary biliary cholangitis (PBC), DF5112 is being developed as a potential disease-modifying immunotherapy targeting the underlying autoimmune component of the disease. DF5112 is currently in Phase I clinical development, with the ongoing study evaluating its safety, tolerability, pharmacokinetics, immunogenicity, and pharmacodynamic effects.
