Leber Congenital Amaurosis Overview
Leber congenital amaurosis (LCA) is a genetically diverse group of early-onset retinal dystrophies, usually presenting in infancy with severe visual loss, nystagmus, sluggish pupils and photophobia. Variants in CEP290, GUCY2D, RPE65 and CRB1 disrupt retinal function; inheritance is usually autosomal recessive, and severity varies. Care includes low-vision rehabilitation, educational support and genetic counselling. Voretigene neparvovec (Luxturna) is approved for confirmed biallelic RPE65-associated retinal dystrophy in patients with viable retinal cells, benefiting a defined subset. Other approaches are genotype-specific: ATSN-101, an AAV gene therapy for GUCY2D-LCA, is in Phase I/II testing; sepofarsen, an antisense oligonucleotide for a CEP290 variant, is in Phase III; EDIT-101, a CRISPR-Cas9 treatment for the same variant, showed early proof of concept, but enrolment was paused in 2022. Genetic heterogeneity, photoreceptor loss, ocular delivery and durability remain key challenges.
Key Highlights
- In the United States, LCA prevalence cases are projected to increase from 59,596 in 2026 to 65,506 by 2036.
- LCA is a rare inherited retinal disorder causing severe visual impairment from early childhood.
- Progressive vision loss and limited treatment options create a significant disease burden.
- Significant unmet need remains for therapies that preserve or restore vision.
